Certified Associate Application

In accordance with the terms and conditions stated below, I hereby submit my application to become a CGF Certified Associate. I understand and agree as follows:

1. I am of legal age in the State in which I enter this Agreement.
   
2. This Agreement shall become binding upon its receipt and acceptance by Charitable Giving Foundation.
3. I understand that upon approval and acceptance of this application, I will be an independent subcontractor, not an agent, employee, partner, or franchisee of Charitable Giving Foundation. I will not be an employee for the purpose of any federal or state law including but not limited to the Federal Unemployment Tax Act or any State Unemployment Insurance Act, Worker's Compensation, Disability Insurance, Employment Training, or Employee Security Act. I further understand it is my responsibility alone to pay all Local, State, and Federal Taxes as required by law including but not limited to Social Security, Medicare, State Disability Insurance, and Income Tax Withholding.
4. I understand that any fees paid for services rendered shall be paid in accordance with the Charitable Giving Foundation Compensation Plan.
5. I understand that if my relationship with the Charitable Giving Foundation is terminated for any reason, compensation will be paid to me only from those fees paid to the Charitable Giving Foundation for the quarter in which my termination occurs prorated to the date of my termination. I further understand that under no circumstances am I guaranteed any specific income, profit, or success.
6. I understand that I may use only official Charitable Giving Foundation literature in promoting the company and its products and services. 
   
7. I understand and agree that I will make no statements, claims, warranties, or representations concerning Charitable Giving Foundation that are not published in official Charitable Giving Foundation promotional materials or websites.
   
8. I understand that I may terminate this Agreement at any time by sending written notice.
9. I understand that acceptance of this application does not constitute the sale of an agency, franchise, or distributorship, nor am I acquiring an interest in a security.
   
10. I understand that my status as a CGF Certified Associate is subject to be annually reviewed. I further understand that my failure to renew the Agreement shall be the same as my written termination of it. 
11. I understand that in the event of my death, any fees due to me will be paid to my heirs or estate.
12. As an independent subcontractor, I agree to safeguard and promote the reputation of the charitable Giving Foundation. I agree to refrain from all conduct that might harm the reputation or the marketing of their training resources to churches and other organizations. I will not misrepresent the Charitable Giving Foundation in any way. I will conduct myself in a professional manner, avoiding all discourteous, deceptive, misleading, unethical, or immoral practices.
13. This Agreement shall be construed and governed under the laws of the State of California.
14. Anyone who became a CA prior to Jan 1, 2020, MUST update their CA agreement by reregistering with CGF using this form.
15. Anyone who is NOT currently registered is no longer eligible to receive funds from CGF without signing this agreement. There is NO charge and NO exceptions.
